What would you do? Modified panelboard

Looks like the EC punched a hole in the wrong place, twice and decided to fill those holes with a piece of another panelboard which they screwed into place. This was a replacement in a condo, like for like.

Would you require a new panelboard to be installed?

Well he certainly isn't demonstrating "good workmanship practices", I would certainly ask him to replace a panel board and see what his response was. I bigger concern would be if you screw this up what else would he screw up later on in the project.
 
Could you could fail it by saying the locknut doesn't have full contact with the housing? Does a manufacturer have any "listed" repair procedures for this kind of thing?
