Accessible Facilities Inventory Forms

I am looking for inventory forms for accessible faciliites either in the PROW or on Private Development. Specifically forms or methods on how to assess/document existing facilities and their compliance (e.g., curb ramps).

Any input would be helpful. Thanks in advance.

Thanks mark. I managed to dig up this toolkit/checklist in the fifth, sixth and seventh installment versions under Appendices 1 and 2 and Chapter 6 addendum: Title II Checklist.

Here's one that the California Department of General Services (DGS) uses: http://www.documents.dgs.ca.gov/resd/RELPS/leaserequirements/consultant%20checklist.pdf. It's not only for the PROW but throughout the building, site and adjacent PROW. It's VERY detailed and thorough - use and/or abuse at your own discretion.
