NFPA 285 Wall assemblies & continuous rigid insulation

Can someone help me get my head wrapped around how NFPA 285 relates to rigid insulation and the relative location of gypsum based sheathing?
Wondering if the gypsum based sheathing could potentially be placed on either side of the insulation (against stud side or at exterior face) and still pass the test.
Also wondering if there is a list of when NFPA 285 is required to be met. Heard today that it didn't apply to wall assemblies less than 2 stories......
