3rd Floor Dwellings?

I know this has been discussed before in many different variations, but I can't seem to summon up the precise search terms, and besides, I'm lazy.

Was there ever a code that existed (30 - 40 - 50 years ago?) that would have permitted a conversion of a single-family structure or duplex into a multi-unit dwelling structure, and creating a 3rd floor apartment that has only one means of egress?

There seem to be a number of these around, and without the benefit of good department records to know if the conversion was properly permitted, I'm wondering if there's any possible way they could have been done, at some time in the past, legally?
 
1970 BOCA allowed one exit --3 stories plus attic 2400 sq ft per floor 6 families 2 hr exit enclosure with 3/4 hr openings section 611.3

Could also be with outside stair with 3/4 hr window and door protection
 
The Virginia Fire Safety Regulations, which were in effect before Virginia adopted BOCA in 1973, allowed a single exit for any story not over 4000 SF. A lot of jurisdictions didn't have building codes 50 years ago, and a few still don't.
