Do existing apartments, permitted in an era before IAQ codes, need to have IAQ systems added when a new or different heating/air conditioning system is added to the apartment?

Probably not....They had ventilation when they were built in theory, an HVAC swap just should not make it worse....A change of occupancy and/or Level 2 alteration might kick it up a bit...
807.1 Reconfigured or Converted Spaces
Reconfigured spaces intended for occupancy and spaces converted to habitable or occupiable space in any work area shall be provided with natural or mechanical ventilation in accordance with the International Mechanical Code.
Exception: Existing mechanical ventilation systems shall comply with the requirements of Section 807.2.

Logically, a building envelope upgrade would be the trigger for ventilation upgrades.
