I was told by a plan checker that the Bobrick B35745 Combo Unit was un-acceptable per CBC 11B 604.7.2 as it is mounted behind the grab bar.
Isn't that what it is designed to do?
 
imho … as long as you have the tp roll at the right height for vertical clearance to the grab bar, and the 1/4” protrusion of the face of the dispenser doesn’t affect the horizontal clearance … no problem.

if you read the spec sheet for the dispenser, they recommend a different model number for accessible installations to maintain grab bar clearance.
 
The newest interpretation in California does not allow us to use the B-35745 Combo unit any longer. Bobrick created the B-3091/3092 to meet the standards.

The reasoning given was that the flange around the unit encroached into the required 1-1/2" clearance at the grab bar by 3/16". Whether you agree with it or not, this is what needs to be used in California.
 
The toilet seat cover tissue dispenser is also part of the issue. You need to have a clear floor space 30x48 side or forward approach and have the seat cover dispenser within reach range. The toilet is obstructing the required clear floor approach space. MtnArch is also correct about the flange behind the grab bar reducing the 1.5" finger clearance. I usually mount the SCD on the wide side wall of the toilet stall or compartment. That way it can be a surface mounted unit and should have the 30x48 clear floor space to approach it.

Operable Parts (§309) Operable parts must have compliant clear floor space for a forward or side approach. They also must be located within accessible reach range and usable with one hand and without tight grasping, pinching, twisting of the wrist, or more than 5 pounds force.
Water Closets [§213.3.2, §604]
Criteria for water closets address location, clearance, seats, grab bars, flush controls, and dispensers. Water closets can be arranged for either a left-hand or right-hand approach.
ADA Chapter 6:
The required clearance provides space for approach and transfer to water closets. The water closet, associated grab bars, toilet paper dispensers, seat cover and
other dispensers,
coat hooks, shelves, and sanitary napkin disposal units are the only elements that can overlap this clearance (§604.3.2). Other elements and fixtures cannot overlap the required water closet clearance. 2010 ADA reference to the 1990 ADA - 4.27.2 Clear Floor Space. Clear floor space complying with 4.2.4 that allows a forward or a parallel approach by a person using a wheel-chair shall be provided at controls, dispensers, receptacles, and other operable equipment
