Box extension used as an electrical box

Review the picture below; two 4x4 box extensions attached to a Caddy bracket with a 4x4 metal cover as the back and on the other side is the switch and receptacle.

To me this installation has the following issues;
  • Box extension being used as an electrical box, since the extension is attached to the Caddy bracket and not a electrical box
  • Metal cover will not be accessible since gyp board will be installed over
  • Caddy bracket not installed per manufacture's instructions

Would you allow this?

Metal boxes in dry locations are not required to be listed; all we have are the construction specifications in NEC Article 314 Part III. So if the metal parts cobbled together meet the requirements of Article 314 Part III, the above is not an NEC violation.

What is behind the metal cover? If there are no splices, and everything can be accessed from the open part, I would not have a problem with this install. Burying splices I would have an issue with.
