Brain Teaser: Electrical clean out.

I started a job today replacing a bunch of parking lot poles. I was doing conduit routing and circuitry research and knew there was a hand hole somewhere I couldn't find. I walked over this clean out a dozen times, but in total frustration figured what the heck. Name the violations.
