New California School gym with flat roof with curb mount skylights. What are the code requirements for fall protection?

I had a kid who served his entire apprenticeship under me, when he earned his journeyman's card he was immediately hired as a superintendent by a non-union contractor for more money than he would have received had he stayed as a journeyman with me. I read in the paper a few years later that he was building a gymnasium, at the end of the day as they were cleaning up he picked up what he thought was a loose sheet of plywood on the roof to take it to cover an opening, unfortunately it was covering a hole and he fell to his death, nobody had nailed it down or written the required lettering on it, my foreman's opinion was that he was the best apprentice that we ever had, he left a wife and two young kids.
