We can get news like this on a regular basis and they all have the same thing in common, multiple injuries.

Are these design or maintenance issues?

The most common causes seem to be improper attachment and/or improper maintenance.

Note the woman's description, "the deck swayed out and then back". I get more grief over trying to enforce lateral bracing than any other single issue.
