Help interpreting cut and fill slope setback diagram

Hi,

I wonder if someone might help me interpret the following cut and fill slope diagram below? I understand that the "a" distance is property line to toe of slope. "c" is face of lower structure to toe. "b" is top property line to top of the the slope. However I don't quite understand what "d" is supposed to represent from the face of the top footing. According to the supplied table "d" can be 5, 7 or 10 feet based on the height of the slope (H) being <10, 10-30 or >30 feet. At what point of the H is the "d" supposed to be measuring to?
