The HD is fastened to an anchor bolt and the HD anchor is next to the stud. The slotted plate washer requires a cut washer over the slotted washer.

Depends on your seismic/wind zone.. I don't believe the code requires the slotted washer is required everywhere.
 
If you're going to use the slotted washer, you should put the cut washer on top. The code only mentions the slotted washer in the seismic zones, but the designer could use the slotted washer for wind resistance purposes.

I will have to ask Simpson if the slotted washer (sans cut washer) provides the same uplift as a standard cut washer.
