How to install a cleanout for a shower drain??

UPC required horizontal branch drains greater than 5 feet to have a cleanout. How do you put a cleanout on a shower drain?
 
Check out 707.4 Exception 3. "Excepting the building drain, it's horizontal branches, kitchen sinks, and urinals, a cleanout shall not be required on a pipe or piping that is above the floor level of the lowest floor of the building."

So how about a shower drain that is in a basement slab? Seems the only way to snake it would be through the trap, which wouldn't be ideal.

The trap is a common way to snake fixtures. I see how your situation doesn't meet the technical terms of the exception I quoted, but I can't imagine requiring a separate cleanout in the scenario you describe. Is this a new fixture going into a location that never had a drain before? basement drains can be problematic for other reasons. Are you needing to install an ejector pump? Are you now below the level of the "next upstream manhole"? Check out 710.
 
You can always install a cleanout downstream of the shower.

Oatey makes a floor drain with an integral cleanout, but I'm not sure it's suitable for a shower.
