Don't have book with me, but here goes

dissimilar metals connected under one terminal

double conductors under one terminal/screw, need to check listing

grounding conductor and grounded conductor under same screw terminal

grounded conductor nicked near terminal block

sheathing on ungrounded conductor not removed enough (just guessing at this point)

bond screw not green?
