Isolated Ground Receptacles

Two questions for everyone:

1) Can an insulated grounding conductor for an isolated grounding type receptacle pass through 2 sub panels without bonding before it gets to the receptacle it serves?


2) If a drawing shows 12/2 MC cable feeding a circuit that is a quad box with 2 duplex isolated ground receptacles, how is it possible for them to bond the box and have an effective isolated ground?

I hope this leads to some quality discussion.
 
1) Good answer. Yes, you are correct as usual (but not always).

2) Yes, that is the best way if they are not going to use conduit. I'd probably do it that way.

3) 2019 because I just started a plan review for a new commercial building that had the IGR spec'd out.
