Mounting height for NEMA4 enclosure

The code requires a maximum height of 6'7" to a breaker handle from the floor to allow for ready access.

If that breaker is within a NEMA4 enclosure that requires tools to open hardware along all edges of the door; would the top edge of the door not have to be within 6'7" of the floor as well? What is the point in having the breaker within reach if you need a ladder to open the door? If there a code section that would support this opinion?
