NEC 250.94 and commercial work/temporary services

Everyone seems to not have a problem with 250.94, although some of the products, including home-made ones have been interesting to say the least. I am curious, however as to how everyone deals with this on commercial jobs where the low voltage stuff may be very far away from the service. The code does not say that the intra-system bonding terminal has to be close to the area it is going to serve and even with some residential, we see it placed inside, next to the panel, no where near the location of the low voltage.

With that being said.

Do you always install a listed connector for compliance on all commercial jobs?

When do you not install one?
