No cleanouts above the first floor required?

UPC 707.4 exception 3: "Excepting the building drain, its horizontal branches, and urinals, a cleanout shall not be required on a pipe or piping that is above the floor level of the lowest floor of the building."

If I have a 2-story home with a crawlspace do I need any cleanouts above the crawlspace? Example: would a 10' horizontal drain pipe from a shower that runs through the joist bay of a second floor need a cleanout?
 
Typically, the building drain and its horizontal branches are below the first floor. In the scenario you describe, the first floor is the lowest floor of the building.

Just to be safe, I would double check the adopted version of the UPC in your jurisdiction because mine reads "Excepting the building drain, its horizontal branches, kitchen sinks, and urinals, a cleanout shall not be required on a pipe or piping that is above the floor level of the lowest floor of the building."

The logic (as it was explained to me) is that most other fixtures act as their own cleanout. You can take the P trap off a lavatory, you can run a snake down a shower/tub drain/toilet/etc...
