Starting with the fact that the prints show a 200A service and you are connected to a metered 60 feeder, you are not ready, but for other reasons. What violations do you see?

 
I don't believe this is a code issue as much as it is just a general quality of workmanship issue. Sad display of our current industry standards.
 
Somebody dropped something heavy on the plastic conduit and broke it ... but that was existing conduit and it’s not in his scope. That’s my story.
Looks like more breaks installed than on the schedule on the door.

Yep, the broke it while removing equipment and what they broke is the feeder conduit to the newly installed piece of equipment. Not going to get a pass for that.
