Pivot door

I recently read an article or post about a wide single pivot door, but I can't find it. It addressed this issue in some manner, but again, I can't recall what the solution was.

Consider a 5' wide pivot door. Maximum leaf size is 4' per IBC 1010.1.1. Neither the code nor commentary address whether that is specific to side-hinged doors or pivot, it just says "swing" doors. My question is two-fold:
(1) Is the 4' max. for the door in any configuration, even if a balanced pivot door or would the leaf be measured to the pivot?
(2) if measured to the pivot, is there code addressing the huge pinch point left between the 4' pivot point and the 5' jamb?

I searched idighardware but couldn't find the info, even though I think her input was included. Any guidance is appreciated.
 
Interesting, 4' max leaf (to pivot?) 32" clr opening at 90 degrees, seems it could work but what of force to move it?
 
Assuming it is balanced, my guess is the force would be fine, maybe even better than a hinged door.

The commentary is vague, only says the reason for the 4' limitation is that they are too difficult to handle. The article I read made a good point about the pinch point. Imagine junior running through it when mommy opens it and it closes. I don't think there is a code for what appears to be a bad idea.
 
I find balanced pivot doors to be very awkward. They swing funny, and the opening is less than you expect.
