Provide access to roofs with only drains?

Does the code require access to roofs which have no mechanical equipment just exhaust pipes and drains? We have a 26' flat roof that we don't want to increase the parapet height for guardrails and a 47' spherical shape roof with drains at 42' we hope can be maintained from a ladder or crane if they get blocked.
 
If the building is four stories or more, then access to the roof is required whether or not mechanical equipment is provided. Otherwise, there are no requirements for roof access.
 
Best practice to do so, consider interior access hatch and harness tieoffs.
Stuff accumulates up there.
 
With no knowledge of the physical constraints it is easy for me to armchair comment but.....If the idea is to reach a drain that's 42' above grade it will require a $1000.00 ladder and young person that's willing to climb it.......or an expensive equipment rental.....each occurrence....or a couple grand towards an access.
