Raised door thresholds?

This is more of a design question than code question which I can't find easily googling. When is a raised threshold required at a door? What about a threshold that seals a door? When does a panic door need to have a sealed threshold at either an interior or exterior condition?
 
I'm not aware that there is ever a requirement to have a raised threshold at a door. There are code limits on the height and slope of a raised threshold if you have one. You might want to have one for exterior doors as part of the weatherstripping scheme, or to keep water from blowing under, or as part of some sort of containment system. Fire and smoke doors also have a maximum space allowable under the door, but aren't required to have a raised threshold. Alternatives might be an automatic door bottom that can provide weatherstripping.
 
"It depends" on the purpose, intended use of the door (interior/exterior) as previously mentioned.
Doors are intended to "secure" openings for various reasons
