In what situation, a room is located off a hallway? And in what situation, a room is not located off a hallway?

I think you might be placing the emphasis on the wrong part of that phrase.

It is providing a minimum doorway width of 610mm where the door is located in a hallway that only needs to be 710mm wide (see 9.5.10.1.(1)).
