The Same Electrical Violations Over and Over

I thought it was no colored tape? Or a torque wrench wasn't used?
 
Oh..Oh... missing hub screws on lower panel!

Also our POCO grounds at the pole now, very seldom do I see a GW coming out of the meter socket anymore.
 
The offset that’s been forced into the hub is a 110.3(B) violation.
 
Chris is that your good screwdriver? Isn't that suppose to be put back on the pegboard wall where it's outlined in white paint?
