Kitchen Refrigerator on GFCI

Does the receptacle for the refrigerator have to be GFCI protected if it is within 6' of the edge of the kitchen sink?
What if the manufacturer's installation instructions recommends the fridge not be connected to a GFCI?

What about if you put a GFCI receptacle behind a fridge, it is still readily accessible?

Doesnt the manufacturers literature govern?
And how could it be readily accessible if behind the fridge? You would have to roll it out to reset. And that assumes the levelling feet have not been extended, over-riding the wheels. And it assumes the morons who laid the ceramic tile took the time to pull the fridge and tile under it, not trapping it in place like the dish washer.
