Yes the code requires that whomever the door is open for also has access to the bathroom. Most businesses have doors that are open to people passing by. If the business restricts those who enter based on some other criteria then all those that are permitted entry must have access to the bathroom. Let's not get hung up on the term "public". The people who have normal access to the business have access to the bathroom located in the business unless there is one provided for outside the tenant space (as in a mall).
Read the plumbing code, it doesn't restrict the requirement to "customers".
403.3 Required public toilet facilities. Customers, patrons and visitors shall be provided with public toilet facilities in structures and tenant spaces intended for public utilization.
SECTION 202 GENERAL DEFINITIONS PUBLIC UTILIZATION. In the classification of plumbing fixtures, "public" applies to fixtures in general toilet rooms of schools, gymnasiums, hotels, airports, bus and railroad stations, public buildings, bars, public comfort stations, office buildings, stadiums, stores, restaurants and other installations where a number of fixtures are installed so that their utilization is similarly unrestricted.
