When you see something like the photos below for a 3-phase system during an inspection inside the electrical room for a generator installation, how do you handle it?
This is clearly a very dangerous situation as the falling flakes of metal are falling into live parts of a 3 phase system. Now, I know this is a maintenance issue but it is also a clear violation of the NEC 110.12(B) and if you are acting in the capacity where it is your job to protect the public and property owners, you should write this up in my opinion, requiring a repair in this case, it would be a replacement.
How would you feel if you found out that a maintenance worker was sweeping the room out when this failed and was severely burned after you let it go?
