The number 1 thing here for failing a plan review is the architects and engineers forget to stamp the plans. The number 2 thing is the wrong codes on the plans. It will be interesting to see what the inspector will do after an architect or an engineer passes a plan review that is not stamped or is designed to the wrong code.
You inspect to the code, even if the plans are wrong. We do it every day. RDPs cannot circumvent the code and it is your job to inspect to the codes.
UNLESS the accepted plans contain 'above code' elements, of course.
