If the tables call for 2-2x8 header, do they need to be nailed together, or can I space them with foam insulation to match the depth of my 2x6 wall?

They need to be nailed as one member...insulation to the warm side. You can add a layer of 1/2" osb or plywood between to make up the difference in thickness though.
 
Is there any information in the code that states this?

There is provision in the code for a 'box header' that is similar to your question but the construction is not the same.
