How does a 13 story internal stairwell collapse?

Top one let loose and the rest collapsed under impact. I wonder if they are pre-cast or cast in place

Sad, very, very sad. If I were the engineer or special inspector I would be very nervous right now.

At least a couple mistakes I believe....This will become more and more common I'm afraid...

It appears that what we have here is a prefab stair with weak connections to adjacent vertical surfaces. As to why it failed, maybe pour concrete mix of improper rebar imbediment, but why and after how long in place?
