Is this isntallation correct?

My concern is going into the end grain on the versulam.

I think they should have used a sidewinder. BTW this is 2 versulams nailed together so it barely meets the 2" nominal requirement. As you can see it is not in the center of the 2" versulam
 
My question would be the load and more importantly the drilling of the LVL.
 
Because it is close to the support I would not expect the beam to be a problem.

The "end grain" may not be a problem since the laminations are glued together. This is a place where one should check with manufacturer's recommendations.

Of more of a concern is how is the rod attached. Are we talking about a through bolt, a lag bolt, or a bolt in epoxy? If we are talking about epoxy then they type of epoxy should be suitable for continuous loads.
