Let is Slide or Not a Violation

Is that back-to-back box extensions with plates on both sides, or just back to back 4x4s? I think a pic from other side would reveal a lot.

Funny seeing smurf. I've only ever seen it on projects in Florida.
 
Sheetrock screws and simpson joist hanger screws securing the crossbar to the studs. Missing screws on the mud ring. Nail plates for the smurf?
