Maximum Size Opening in Walking Surface

What is the maximum size opening allowed in a walking surface for a landing or any walking surface?

All I can find is in ANSI A-117 at 1/2" but hat seam too large to me.

302.3 Openings. Openings in floor surfaces shall be of a size that does not permit the passage of a ½ inch diameter sphere...

I agree.. look at the heels on the shoes some women wear... a 5/8" heel can still jam itself into a 1/2" opening and she'll get hurt. There's no good solution (other than outlaw high heels.. which I would support.. they hurt my feet).
 
Openings in working/walking surfaces that are not on an ADA-accessible route or a means of egress, such as mechanical platforms, catwalks, fly galleries, etc. are governed by OSHA 1910.23(a)(8) as "floor holes."

For accessible and means of egress routes that need open grilles, there are micro-gratings and T-bar grids with openings under 1/4 inch (6 mm) that are safer for stiletto traffic.
