Monolithic slab with integral footings

You haven't indicated your understanding of
the potential forces at work here:]

1. Frost heave
2. Loading to support 2 stories
3. Type of soil? bearing value, expansive? Water table if any, depth?
4. Type of concrete
 
ICE, I had to look up syntax, I had hear of the word, just never used it in a sentance.

Oh... one #5, not a good frost footing does it make, are we adding up-rights? or ratcheting up the concrete mix specs? Going with the minimums I suspect?
