Where are you required to make the connection between the equipment grounding conductor (EGC) and the neutral conductor at a 208/120V, 3-phase, four-wire service?
 
250.24(B) in the 2014 NEC tells us that the equipment grounding conductor, the grounded conductor and the service disconnect enclosure shall be bonded together "within the enclosure for each service disconnect in accordance with 250.28."
 
250.24(C) in the 2014 NEC. Grounded Conductor Brought to Service Equipment. Where an ac system operating at 1000 volts or less is grounded at any point, the grounded conductor(s) shall be routed with the ungrounded conductors to each service disconnecting means and shall be connected to each disconnecting means grounded conductor(s) terminal or bus. A main bonding jumper shall connect the grounded conductor(s) to each service disconnecting means enclosure.
