Where does it vent? Maybe the 90's vs. cleanout.....tough to say without more info....306.2.2 Rock removal? Ohhhh....and I would normally see some sort of test at a rough plumbing.....

When the cap is screwed on to the tee, it appears to be

below grade.......Do not see any thread lubricant or tape

on the cap [ Section 705.2.3, `06 IPC ].

Always wondered how short the horizontal arm of the drain connected to the tap has to be before it becomes an S trap.
 
There is a Y-1/8th bend, which is the sweep they need for 90 degree horizontal changes of direction. They just need something like that next to the flat san-tee. Plumbing isn't my strong suit so I always ask that the pipe be completely exposed on a firm bed with a ten ft. head of water. That means no dirt at the sides. The glue job looks pretty sketchy. And that looks more like a Q-trap than an S-trap.

Is a Q trap prohibited too??? Or is a Q trap just an O trap with a low inlet heel?
 
Dan has it right.. the wye needs to be on the flat, and hopefully there is a proper vent downstream.

Those do not look like 3" long sweeps, as required for horizontal changes in direction.
