For years we've provided above-toilet cabinets on some projects, but I don't think that what we've been doing is ADA-compliant. According to ANSI 117.1 307.2, we can't have anything lower than 6'-8" that protrudes from the wall more than 4". Is there any other provision in the ANSI that would allow this above-toilet cabinet?

I think a cabinet below 80 inches encroaches on the required clear floor space for the toilet. On a different topic, the mirror height requirement is to the reflecting surface rather than the decorative frame.
 
Under the 2010 ADASAD there is an Advisory 225.2 Storage.
Types of storage include, but are not limited to, closets, cabinets, shelves, clothes rods, hooks, and drawers. Where provided, at least one of each type of storage must be within the reach ranges specified in 308; however, it is permissible to install additional storage outside the reach ranges.

So, if you provide "accessible storage" somewhere else, this cabinet can be placed there.

2010 ADASAD 603.3 Mirrors. Mirrors located above lavatories or countertops shall be installed with the bottom edge of the reflecting surface 40 inches (1015 mm) maximum above the finish floor or ground. Mirrors not located above lavatories or countertops shall be installed with the bottom edge of the reflecting surface 35 inches (890 mm) maximum above the finish floor or ground.
 
If you have a tank toilet it might be OK as long as you have accessible storage elsewhere, it doesn't project beyond the depth of the tank, and is at least 18" above the grab bar (this dimension is from the handrail in a recess in another thread). It should probably be tight to the sidewall, or else narrower so it doesn't project far beyond the side of the tank.

seems reasonable enough, but I know of no code language that would allow you to install an object above the toilet that does not project as far as the tank
