The J-L Bolt Debate

How could an anchor of any type be pulled out of concrete before the wood framing failed?

I was a carpenter on a wood framed, four story, hotel in a Ca. coastal city. There were massive anchors around the elevator shafts. The material was 1.5" all thread with steel plates held on with nuts, The rods were about eight feet long into a huge block of concrete. One would think that eight feet was long enough ... but they ended up short. No problem, they were un-threaded until they reached the correct height above the concrete.
 
The t bolt was just an idea that popped into mind that would possibly offer more resistance to tension. Something simple for use by untrained workers like a j bolt. Obviously lots of ways to provide more resistance but those might require more of a thoughtful and precise install than the current j bolt.
