When a machine is moved without a permit

This is the result when a commercial company moves a 3 phase machine to a new location without hiring an electrician, does not pull a permit or gets an inspection. What violations do you see here?

What you don't see is:
*20' of FMC already damaged laying on the floor with no connector on the machine end.
*30' of SO cable from a junction box, across the bottom of a garage door to the disconnect
*A junction box not properly grounded without a cover

BTW, this is a 3 phase 240v Delta with a 208 high leg on the C phase (white)

I like it when I can discern the fuse amperage rating. The green screw appears to be too long for the application.....and how about the high leg conductor with white insulation.

The wire looks like what you find when a cord is stripped of the outer sheath. Is that what happened here?
